Data source + refresh cadence. Every licensed facility on this page comes from the New York State Department of Health (NYSDOH): the Health Facility General Information roster (operating certificate, address, phone, operator, first-licensed date), the Adult Care Facility Quarterly Statistical Report (2026 Q2 certified capacity by ALR, EALR, SNALR and ALP), and each facility's inspection and enforcement record on NYS Health Profiles. What is the difference between an Adult Home, Enriched Housing and an ALR?

An Adult Home and an Enriched Housing Program are New York's two base adult care facility licenses; ALR is an added license that requires more services and lets the community call itself assisted living.

Enriched Housing is apartment-style, often with kitchenettes; an Adult Home is more congregate. 12 Buffalo-area communities hold no ALR license; several of them deliver assisted living through Medicaid ALP beds instead. Our adult home vs. enriched housing guide covers the difference.

Is assisted living the same as memory care in the Buffalo area?

No. Memory care in New York means SNALR certification, and only some ALR communities have it.

Assisted living (ACF with ALR / EALR)Memory care (SNALR)
What it isAn Adult Home or Enriched Housing Program licensed by NYSDOH that also holds Assisted Living Residence licensure; EALR lets a resident stay as medical needs grow.A Special Needs Assisted Living Residence certification for residents with dementia, added on top of an ALR license.
What it isn'tNot a nursing home: no 24-hour skilled nursing under 10 NYCRR Part 415.Not a separate license, and not proven by a brochure that says “memory care” — the SNALR certification is the proof.
Who it fitsSomeone who needs help with bathing, dressing, medications and meals but is medically stable.Someone whose dementia makes wandering, exit-seeking or unsafe judgment a daily concern.
How to check itThe ALR and EALR bed counts in the NYSDOH quarterly report (shown in our tables). The SNALR bed count. Zero SNALR beds means the building is not certified for dementia care.

Which Buffalo-area assisted living communities accept Medicaid?

12 communities hold Assisted Living Program (ALP) beds, New York's Medicaid-funded assisted living; the ALP entry in the Certifications column identifies them.

ALP beds are limited and fill quickly. See how Medicaid pays for assisted living in New York.
